What is Routine Home Care Hospice Service?

This is the most common level of hospice care.

Routine home care includes, but is not limited to: Nursing, hospice aid, spiritual care and Medical Social Work services. Patients may receive Routine Hospice Care in their home or where they currently call “home”—in long-term care, assisted living facility, independent living, or in adult foster care.

  • Medical equipment needed for optimal care covered

  • Available based on your schedule and needs

  • Palliative Physical Therapy for teaching and comfort available

How We Assist

Being locally owned and a part of our communities, the needs of the patient come first, determining the number of visits from hospice staff members vs. industry standards. These needs are established and outlined in a plan of care formed by the routine home care hospice team and the patient’s physician.

The care plan serves as a guideline to assist all those serving the patient with care. At this level of care the patient also has access to an on-call hospice nurse twenty-four hours a day.
