What is Routine Home Care Service?
This is the most common level of hospice care. Routine home care includes, but is not limited to, Nursing, Hospice Aide, Spiritual Care Providers and Medical Social Work services. Patients may receive Routine Hospice Care in their home or what they “call home”—in a long-term care, assisted living facility, independent Living in adult foster care.

How We Assist
Being locally owned and a part of our communities, the needs of the patient come first, determining the number of visits from hospice staff members vs. industry standards. These needs are established and outlined in a plan of care formed by the hospice team and the patient’s physician.
The care plan serves as a guideline to assist all those serving the patient with care. At this level of care the patient also has access to an on-call hospice nurse twenty-four hours a day.
